Internet trends and social media have influenced what we think we want. Trends dominate the aesthetic space. We simply cannot give each and every patient the same treatment based on a trend. We have all seen the telltale signs of fillers: Everyone has the same cheeks, lips and jaws.
We want instant gratification. We want to fit in, be on trend and relevant. We see photos on instagram, but it’s important to remember that a 2D picture and how the lips look in person in 3D in motion from all angles is different. Before and after are very different than in person and in motion.
Fillers do not break down as quickly as we once thought they did. With the increased use of ultrasound we can accurately understand exactly where and how much filler is in the skin. After doing fillers repeatedly over time there is an accumulation of product and cumulative effect. Sometimes this is good and other times, not. Overfilling and migration happen. The look is doughy, overfilled and unnatural. Fillers absorb water, swell and occupy space. We must account for this.
In the lips, after multiple treatments the lips can become distorted, not look right and not function right. When things don't look right the brain registers this unnatural look. The brain detects extra pressure, turgor and extra hydration that does not match the rest of the face.

Slow and steady wins the race. Hyaluronidase works very quickly. Like fillers we do not want to over dissolve. We want to assess over the course of a couple to several sessions and in stages. No carpet bombing. We do not want to shock the system physically and psychologically. There is a lively discussion about hyaluronidase making things worse and destroying collagen. I think what happens is that we have gotten so used to looking in the mirror and when the filler is removed, we see deflation, wrinkles and texture changes that are actually caused by the filler itself. Oftentimes, we don’t need to remove all of the filler, but rather take it down a notch, about 30-40%.
